Transtracheal jet ventilation in patients with severe airway compromise and stridor.

چکیده
Editor—We read with interest the article on transtracheal jet ventilation (TTJV) in patients with severe airway compromise by Ross-Anderson and colleagues. We agree with the authors regarding the enormous value of the pause pressure protection offered by automated jet ventilators in the elective setting. Protection from barotrauma is just as important in the emergency obstructed airway. The Ventrain (Dolphys Medical, Eindhoven, The Netherlands) is a new device which offers an alternative approach to the prevention of major pressure-related complications. It was first presented at the Difficult Airway Society meeting in Liverpool in 2008 and is now commercially available from Inspiration Healthcare. It is a single-use TTJV device marketed for use in emergency complete airway obstruction scenarios. It consists of a handheld device with tubing to connect one end to an oxygen supply and the other to a narrow bore transtracheal catheter. It allows not only inspiration but also active expiration by generating suction using the Venturi effect, thus reducing the risk of barotrauma caused by inadequate exhalation. We would be interested in the thoughts of the authors about the place of this device in emergency and elective airway management.

Transtracheal jet ventilation in 50 patients with severe airway compromise and stridor.
BACKGROUND Management of the airway is difficult in patients with pharyngeal or laryngeal pathology caused by malignancy, extensive surgery, or radiotherapy scarring, particularly when undergoing pharyngolaryngeal surgery. Tracheal intubation, with or without fibreoptic guidance, is often impractical because of the severe glottic stenosis and primary tracheostomy under local anaesthesia has bee...

Abstract Background: Transtracheal jet ventilation is an alternative ventilatory approach in airway surgery. The purpose of this study was to compare respiratory efficacy of percutaneous transtracheal with endotracheal, low frequency jet ventilation during microlaryngeal surgery. Methods: Ninety ASA class I-II patients aged between 16 and 70 years undergoing elective microlaryngeal surgery wer...

BACKGROUND Spontaneous ventilation, mechanical controlled ventilation, apneic intermittent ventilation, and jet ventilation are commonly used during interventional suspension microlaryngoscopy. The aim of this study was to investigate specific complications of each technique, with special emphasis on transtracheal and transglottal jet ventilation. METHODS The authors performed a retrospective...
متن کاملTracheal gas insufflation and related techniques to introduce gas flow into the trachea.
Over the past 50 years, a variety of techniques have been developed that have in common the insufflation of gas into the central airway to facilitate carbon dioxide (CO2) clearance. These include continuous insufflation of oxygen, transtracheal jet ventilation, high frequency jet ventilation, transtracheal oxygen administration, intratracheal pulmonary ventilation, and tracheal gas insufflation...
متن کاملTranstracheal high frequency jet ventilation for endoscopic airway surgery: a multicentre study.
Serious complications during high frequency jet ventilation (HFJV) are rare and have been documented in animals and in case reports or short series of patients with a difficult airway. We report complications of transtracheal HFFJV in a prospective multicentre study of 643 patients having laryngoscopy or laryngeal laser surgery. A transtracheal catheter could not be inserted in two patients (0....
